A formal investigation must be carried out when serious misconduct or misconduct has been alleged.
Formal Investigations
A formal investigation will be carried out and discussions held with all team members considered able to assist.
During an investigative meeting involving an employee, where it becomes clear that the situation has escalated to be disciplinary in nature, the investigation must be stopped immediately and the employee advised of the following:
· That this is now a disciplinary matter
· That as a result the employee’s employment may be affected.
· Outcomes may include warnings up to and including dismissal, depending on the circumstances
· That they may have a representative with them at subsequent meetings.
If the employee responds that they wish to have a representative present, then an appropriate timeframe must be given for them to seek representation.
Record of Meetings
Comprehensive file notes and records of statements must be kept throughout the course of the investigation to ensure an accurate record of events for future reference.
